About Happy Hearts and whether it is real

Is Happy Hearts a real company?
Yes. Happy Hearts is a heart health service operated by Happy Heart Care Services, LLC. It also takes part in a Medicare program called the ACCESS Model. You can confirm this by checking the list of approved participants on the official CMS website.
Is Happy Hearts a scam?
No. We understand the question, because real scams do exist. A real program does not pressure you, does not ask for your Medicare number out of the blue, and shows you what is included before you sign up. That is how we work. You can also check us against the public CMS participant list.
Why did I hear about Happy Hearts through Medicare or my plan?
Happy Hearts takes part in a Medicare program called the ACCESS Model, so you may hear about it through Medicare or through a group you already belong to. It is not a random offer out of nowhere.

The ACCESS Model

What is the CMS ACCESS Model?
ACCESS stands for “Advancing Chronic Care with Effective, Scalable Solutions.” It is a model run by the CMS Innovation Center, which tests new approaches to care, for people on Original Medicare. Happy Hearts takes part in it. Because it is a published CMS model, you can look it up yourself on the official CMS website.
Is the ACCESS Model a separate insurance plan?
No. It works within Original Medicare, not as a separate plan and not as Medicare Advantage. You keep the Medicare you already have.

Cost

Is Happy Hearts free?
We say “no cost,” not “free,” because we want to be exact. If you are eligible and enroll with Happy Hearts, there is no cost to you for the program. We show you what is included before you sign up, so there are no surprise costs.
Will this cost me anything? Is there a copay?
If you are eligible and enroll with Happy Hearts, there is no cost to you for the program. If anything would ever cost you money, we would tell you before you signed up.
Does Medicare cover a home blood pressure monitor?
It can depend on your situation. Original Medicare covers some blood pressure monitoring in certain cases. The clearest way to know what applies to you is to check Medicare.gov or ask us directly.

Privacy and your information

Does Happy Hearts sell my health information?
No. Your health information is yours. We do not sell it, and we do not share it without your say. You can read the details on our privacy page.
Who can see my readings?
You can see your own readings in the app. You decide whether to share them, for example with your own doctor. We do not hand your information to others without your say.
Is the Happy Hearts app private and safe to use?
Your information is protected and handled with care once you enroll, under our HIPAA Notice of Privacy Practices. For the full details, please read our HIPAA Notice of Privacy Practices.
How do I delete my Happy Hearts account?
You can delete your account from inside the app, through Profile → Settings → Delete Account. Or email our support team at support@happyheartcareservices.com, or our privacy team at privacy@happyheartcareservices.com, and they will close it for you and answer any questions you have. Once we have your request, we delete your account and personal information within 30 days. Some health records must be kept for longer under HIPAA and state medical record laws. Your doctor and your readings

Does Happy Hearts replace my doctor?
No. Your own doctor stays in charge. Happy Hearts works alongside them, providing care for the time between your regular visits.
Can I share my blood pressure readings with my doctor?
Yes. The app keeps your numbers in one place so you can bring them to your doctor or share them when you choose. Tracking your numbers can help you and your doctor spot patterns and talk about them.

The monitor and the app

Does the monitor need Wi-Fi, a password, or complicated setup?
The monitor works the moment you open the box, with no Wi-Fi needed, ever. To get your numbers explained in plain language and tracked over time, a one-time Bluetooth connection links the monitor to the app. The app guides you through it step by step: a few taps, no passwords, no typing. After that, readings sync automatically. If you would rather skip the app, the monitor still shows your reading right on its own screen.
What comes in the box?
A blood pressure monitor, a charging cable, a plain-language setup guide, and a QR code to download the Happy Hearts app. The app is where your numbers get explained in plain language and tracked over time. It connects to the monitor through a simple one-time Bluetooth pairing the app walks you through.
Do I need to be good with technology to use it?
No. Taking a reading is one button, and that is it. Connecting the monitor to the app is a one-time step the app walks you through, with no Wi-Fi, no passwords, and no typing. It is designed to be simple for people who are not tech-savvy. And if you ever get stuck, a real person can help.
How do I know if I am eligible?
The simplest way is to ask. Leave your details on our contact page and we will follow up to walk you through it. There is no pressure, and no cost to find out.
